How to Order Without Using Your Online Account
If you do not wish to activate your online account at this time, you can still mail your prescriptions to TMOP. Just follow the instructions below:

1. Print and complete the TMOP Mail Order Registration Form (PDF file).

2. Make sure the following information is included on each prescription (any missing information should be written on the back).

• The patient’s full name, date of birth and address
• The sponsor's Social Security number
• The prescriber's name, address, phone number and DEA number
  
  
3. Mail the completed registration form, your prescriptions, and payment to:
